formal complaints to the MCB Hawaii Base Inspector's Office.
Please select a type of complaint from one of the three options: Equal Opportunity: Select this option to file a formal equal opportunity complaint. Your complaint will be forwarded to the MCB Hawaii Equal Opportunity Advisor. We recommend that you visit our Equal Opportunity web page before using this option. Fraud Waste and Abuse: Select this option to file a formal complaint related to fraud, waste and abuse. Your complaint will be forwarded to the MCB Hawaii Deputy Base Inspector. Other Formal Complaint: Select this option to file a formal complaint against an organization, an individual, or service aboard MCB Hawaii, that does not fall into the above two categories. Your complaint will be forwarded to the MCB Hawaii Deputy Base Inspector. To submit this form you must fill in, at a minimum, the two required fields (Type of Complaint and Complaint Details). We recommend that you enter the complaint details in chronological order. After you fill in the two required fields, you must click or select the "submit" button. If you attempt to submit the form without those two fields, you will receive an error and the form will not be forwarded until you correct the error. All other entries are optional; however, if you wish to be contacted or if you desire a response to your complaint, you will need to provide your name and contact information (e-mail address, telephone number, or mailing address).
